School Facility Rental Request Application

Attached is the Rental Request form for all areas of the School District of New Holstein Facilities. Please return the forms to the office in which you would like to reserve a room or to the District Business Office. The form will be reviewed and returned to you granting your request and advising you of any fees that might be associated with the rental. In addition, I have included the Public Use of School Facilities Document which details fees, expectations, etc. If you have any questions, please contact me.
